Clean up Log Out flow

Seeing a few issues with the controller log out button. check out the video I recorded below...

Specifically

• When click the log out button in the controller settings page the controller stays open on the screen
  -> Change this so that when the log out button is clicked the controller window closes

• When I log out of the controller I still appear as logged in in the app/game I was playing until I hard refresh
  -> If we can do something about this we should. after clicking the log out I expect to see the connect button

• It looks like my password manager thinks theres an input after I click log out. this is pretty weird but will probably be fixed if we resolve issues 1 and 2

Bonus points

• Build a log out confirmation screen. This should slide in from the bottom of the controller
