You should always be able to access visits even if the patient table is overflowing with data

Follow on to #527 - how can we make it so you can always access the visit list for a patient even if the table overflows on a small laptop/iPad screen. I think mobile is out of scope here since that would need a different table UI entirely.

Some ideas:

• Include the visits inline in the patient table
• Make the visits button float somehow
