CommandMenuItem is not notified of Command changes so the menu never updates. There is a commit, that attempts to address this, that's part of an open pull request but it's over a year old. Any objections to propagating NotifyOfPropertyChange up to CommandMenuItem so the Menu will get updated?

Enabled ToolBars in the Shell.

Tried making PhxOpenFileCommand to deal with tgjones/gemini#277.
However that is blocked by tgjones/gemini#174.
