Missing url attributes on request traces #123

The URL attribute is not set on request traces, causing all requests that use a catch-all route to appear with the same name in monitoring platforms.

For instance, when using Application Insights:

A solution outside the library would be to use the AttributesFromHeaderFunc but that does not currently work, see #121.
